I remember the first time I encountered this convention: in a Tom Clancy novel, where it was remarked that the President's academic and other titles meant nothing, and he was "reduced" to "Mr." again. It was presented as an esoteric bit of information with which most people are unfamiliar, and which can take getting used to if unexpected, but which ultimately was the correct etiquette and therefore properly respectful.

It need hardly be remarked that Tom Clancy was FLAMINGLY right wing.

These people are absolutely bugfuck.

Point of fact: I am a PhD which means that I am an actual DOCTOR (of Philosophy with a concentration in Southern Literature). However, I do not insist that people refer to me with the honorific and have actually told people to not refer to me as Dr. because I am NOT A PREENING PRETENTIOUS NARCISSISTIC ASSHOLE.
