so i'm interested in this b/c i've been doing ACA work for 5 years and i'm trying to figure out your friend's trap (there are several. if we had a congress that was helpful these would have been fixed years ago).

i assume you all are in a red state? i also assume that 12.5K you are talking about is the 100% of the FPL (this number varies by state but from 0 to 100% FPL you don't qualify for tax credits if you don't qualify for medicaid - unless you're a lawfully present non citizen...). i also also assume the 'doesn't qualify for ACA' means doesn't qualify for subsidies? almost everyone can buy on the exchange, you just pay full price. (being in jail and undocumented are among the only restrictions).

here in IL (expansion state) we work like this: under 16k (ish) you qualify for medicaid. from 16k to 50K (ish) you qualify for a sliding scale of support. if you don't qualify for medicaid and it's not because of immigration status (medicaid kicks in after 5 years with a green card or refugee / cuban / haitian status - FOR NOW), then you don't qualify for price supports on the exchange (in 5 years and probably 2500 applications, i have seen this happen exactly once. but again, i'm not in a red state...)

and yes, many poor people are truly screwed - and esp in red states.

i'm sorry for your friend. i wish i could help. i helped my in laws in NC and was horrified at how regressive it was compared to IL.
