While it may vary by location, 45% sounds hella high, unless there was some extensive p;retrial litigation before settlement. Usually (in my experience) you gather up the medical bills, maybe get a written consultative narrative report from a doctor, toss those at the insurance company, and negotiate a mutually tolerable number. There are other ways to go about it, but the proper approach varies from case, and the above is the commonest and simplest.

No, let your insurance co. lawyers handle things. I hired an outside of my insurance co. lawyer and he did nothing for me beyond recover my rental car fees above what my insurance paid ( father of the girl who broadsided/ totaled my car said his insurance would cover any rental car I had. He was a liar). My insurance co. made sure my medical bills were paid and I got Blue Book value for the totaled car.That’s my experience.
