as AuntPP below says, for sure check with the hospital's financial help (it goes up to 400% of the fed pov level - like the ACA). but i think you should fight the ins company. what grounds did they give for refusing to pay? In IL we have several orgs that will help you fight ins company denials. i've also heard your HR (depending on size of emp) can also fight.
in general and always with ins companies: fight every angle and every decision.

Medicare has a great site that explains the plans. Each different plan gets a letter and the coverage is the same no matter who you buy it from, so buy by cost. Plan F is the best. You can get it with a high-deductible to lower the monthly costs. And don't forget prescription coverage. If you don't sign up when you turn 65 but do later you will have to pay a permanent penalty. They claimed it was a pre-existing condition even though I had not clue it was coming and it had never been diagnosed. Our Insurance Commissioner at the time was a Repub whose donors were largely insurance companies. Someone started a class action suit for similar cases. After 8 years BC settle for about 20 cents on the dollar (and only after demanding tons of documentation that was hard to find years later). Crime does pay.
