I would beg to assert that we are both right, that negligence is the causative agent and precedes every unintentional discharge.

Mom had more than her share of faults, but she was a superb nurse and trained a whole generation of LPNs. Where health issues, such as unwanted babies and diseases were concerned, she was no-nonsense.

Good point, because that is a real "accident," an event that is happenstance and not predictable. That's why I use unintentional instead of accidental. With negligence, often the person who was negligent is not the person who pulls the trigger, and the negligence may have been longstanding until the final element, a child, enters the scene and makes the event highly predictable. This is the kind of enjoyable kibbitzing we used to engage in on the copy desk, while editing stories about unimaginable tragedies.
