This is and has been a real issue. Some people have lungs and hearts that cannot support breathing on their own. Before surgery, patients like that have tests to ensure they can come off the ventilator. That is one reason some surgeries can't be done. But this doctor sounds evil.

She's a radiologist. The last time she was anywhere near an actual patient struggling to breathe on a respirator was as a medical student. She just looks at those nice clean xrays.

Many volunteers mucho danger - no testing.

A legal question on this. Doesn't this 'directive' need to be signed in the current year? When I worked at a hospital, there was often some issue over having the patient sign a new one to bring it up to date.

This is an ugly question. Dad, brother, niece, Gramma has some brain function but won't wake up. Are you going to keep her/him on an expensive breathing machine for a week, month, year? (Even in non-pandemic times this is true.)
