I'll wager this was why the study was conducted in Brazil, where you have a population already taking Ivermectin for it's approved medical purpose. These are so-called 'observational' studies; you don't need to have a hypothesis of it's MOA, just observe if people already taking the drug don't get sick with COVID)
This is a very common way of testing such claims like this. (this was also the basis for clinical studies of hydroxy2000bonercream, I mean hydroxychloroquine...as it's used in the reatment of a number of medical issues you have a good experimental group)
