Outside the US people use it as a news source. And everywhere that it has users it functions as a social site, to meet and talk with people from literally all over the world.
Each instance (that's what they call their servers) handles their own moderation, so it varies. If you don't like the moderation levels at one instance, you can switch to another, and everything about you follows you to the new place.
