The docs I see work for an academic medical center, and while they're paid very well by ordinary standards only the high administrators make up into the millions as far as I know (it's this way everywhere. Administrators see fit to pay themselves handsomely, because otherwise you won't get Top Administrative Talent^(TM).) My wife is not an MD, but does much of what doctors do for a much, much lower salary. Primary care can be just incredibly hard work.
I suspect that if Bezos started paying his employees a fair wage, Amazon's stock would crash and most of his fortune would evaporate.

They're highly paid specialists in private practice. In primary care -- where we really need more doctors -- the rewards are nowhere near as great. I think it's good for doctors to be well-compensated, but the pay scale for certain specialties is nuts -- it tends to attract people who want to get rich as opposed to people who want to be good doctors.
Another anecdote -- my best friend from high school is a pediatrician. He does ok, but is not rich at all. He is also an incredibly skilled and dedicated practioner.
